1. Are you looking for independent retirement living?

a. Yes, but I want someone to take care of snow removal & yard maintenance. b. Yes, but I want meals prepared and light housekeeping. c. Yes, but I would like some health care services. d. Yes, but I need skilled nursing services.

2. Are you interested in purchasing a retirement home or looking for month-to-month rent, with no endowment fees?

a. Yes, I would like to purchase. b. Yes, I would like an apartment. c. Yes, I would like monthly rent with health care services.

3. Do you want/need your meals prepared for you with your choice of entrées?

a. No. b. Yes.

4. Are you able to eat independently?

a. Yes. b. Yes, but I need assistance with opening cartons and cutting meat. c. No, I need physical assistance with eating.
